CSV Importer - blank selection fields after selection

I'm using the latest version of CSV importer (1000) and noticed that when I select the file to upload the field on the form remains blank and doesn't show the selected filename. Screenshot attached to demonstrate. It doesn't appear to affect functionality as the file will still import but looks odd as no confirmation you have selected the right file.

   Mac or Windows?
   Which version of MD?
   After this happens, can you grab help/console and upload that here please?
