Data transfer back to MD

I previously used MD, switched to Quicken and am now considering a return to MD. I have saved a QMTF file and renamed to QIF to import into MD, but the balances are way off. I tried the suggestion to create an initial balance transaction via the Account file, but that didn't work so I started looking closer and noticed that many transactions seem to be duplicates of transfers. Specifically, there are ones that have TXFR in the Check # line and duplicates that have [TXFR], presumably because Quicken links the transactions to another account via some command. How can I easily remove one set of the duplicates without combing through 14 years of transactions?

Also, regarding transfers, what is the process MD uses to create a transaction in one account via another account? i.e. I pay my AmEx from Banking and enter the transaction in Banking as a debit. Must it also be entered into the AmEx account manually as a credit?
